This page provides online transportation forms for parents/guardians, school staff and district transportation coordinators. Use the appropriate form below to submit your request, ask a question or report a transportation-related concern.

Before you begin, please review the following before submitting a form:

  • Address changes: Student address changes must be reported to your student’s school. Do not use the Bus Stop Change Request form to report an address change.
  • One Student Per Form: Please submit a separate form for each student.
  • Need Help? Contact the Transportation Customer Service Center if you are unsure which form to use.

Forms for Parents & Guardians

Request a change to your student’s assigned or approved alternate stop (Note: Report address changes to your student’s school). The RSCO Transportation Office reviews the Change Request Form and either approves or denies it. RSCO Transportation makes every effort to accommodate a request that meets the RSCO Transportation Guidelines. If a change request is denied, families may appeal the decision by following the process outlined below.

Request transportation accommodation for your student due to a documented medical condition. A physician's certification is required.

New to transportation? Use this form to ask questions about bus assignments, transportation procedures, and getting started.

Use this form to request bus information, report a lost item, ask about transportation reimbursement, or submit a general transportation question.

Use this form to request reimbursement when a family is eligible and provides transportation for their student. Reimbursement is based on program eligibility and student attendance.

Report late buses, safety concerns, driver or monitor issues, bus stop concerns, or other transportation-related issues.

Forms for Schools & District Staff

These forms are intended for school personnel and district transportation coordinators only.

Request a bus stop change for a student. The school must update student address changes before transportation can process them.

Request transportation for approved activities that begin before the regular school day.

Request transportation for approved after-school programs, athletics, and other school-sponsored activities.

Request transportation for school-sponsored field trips and educational activities.

Bus Route/Bus Stop Appeal Process

If a change request is denied, families may appeal by following the process below.

1. First Level Appeal: Parent/guardian or a student (18 years of age or older) may appeal a denied change request to the RSCO Transportation Office as follows:

  1. Submit a written request to appeal the change request decision to the RSCO Transportation Office by:
  2. The written request must include detailed reasons for the appeal. The RSCO Transportation Office (RSCO) will review the appeal within five (5) business days and notify the family of the decision in writing:
    • If the appeal is approved, the change request will be implemented within 72 hours; or
    • If the appeal is denied, the parent/guardian or student (18 years of age or older) will be notified by telephone and in writing that the appeal was denied.

2. Second Level Appeal: If a parent/guardian or student (18 years of age or older) is not satisfied with the First Level Appeal decision, they may request a Second Level Appeal to be reviewed by RSCO as follows:

  1. Submit a written request for a second appeal to the RSCO Transportation Office by:
  2. The second level appeal request should include any additional information and/or reasons (if applicable) for the second appeal.
  3. The RSCO Transportation Office will submit the appeal request and supporting documentation to RSCO for review and a final decision.
  4. RSCO will review the appeal within five (5) business days and will either approve or deny the appeal request and communicate the decision to the RSCO Transportation Office.
  5. The RSCO Transportation Office will notify the family of RSCO’s decision in writing if:
    • the appeal is approved, the change request will be implemented within 72 hours; or
    • the appeal is denied, the parent/guardian, or student (18 years of age or older) will be notified by telephone and in writing that the appeal was denied.
  6. The decision rendered by RSCO is final.
